Jentry is the owner and founder of Re-Form and a Doctor of Physical Therapy with advanced training in neurological, spinal, and movement-based rehabilitation. She earned her Doctorate of Physical Therapy from Texas Tech University Health Sciences Center after completing a Bachelor of General Studies with concentrations in Psychology, Business, and Biology. After working across multiple specialty areas, Jentry founded Re-Form in 2015 with the vision of delivering thoughtful, individualized care rooted in clinical excellence.
Jentry’s clinical focus includes spinal cord injury rehabilitation, neurological disorders, and movement and muscle recruitment dysfunction. She is a fully certified STOTT Pilates® instructor and is completing the STOTT Pilates Rehabilitation certification, integrating Pilates-based principles into physical therapy to support spinal health, strength, and efficient movement.
Jentry is a dedicated community advocate and has served on the Turn Center Medical Advisory Board, the Turn Center Board, the Amarillo College Foundation Board, and is currently on the TTUHSC DPT Clinical Education Advisory Committee, and serves as President of the St. Andrews Episcopal School Board.
Erin is a Doctor of Physical Therapy with specialized training in women’s health, orthopedic rehabilitation, and vestibular conditions. She earned her Doctorate of Physical Therapy from Texas Tech University Health Sciences Center and holds a certificate in Women’s Health Studies from Texas Woman’s University, supporting her work with pregnancy and postpartum-related pain.
Erin treats a wide range of conditions, from orthopedic injuries to balance and vestibular disorders. She is known for her thoughtful, patient-centered approach and her commitment to helping individuals regain confidence, comfort, and function in their movement.
Erin has completed comprehensive STOTT Pilates® training in matwork, reformer, Cadillac, and chair, integrating Pilates-based movement into her physical therapy practice to support strength, stability, and long-term wellness.
Lauren is a Doctor of Physical Therapy with a background in kinesiology and a strong foundation in movement-based care. She earned her Bachelor’s degree in Kinesiology from Sam Houston State University and completed her Doctorate of Physical Therapy at Texas Tech University Health Sciences Center.
Lauren is a trained STOTT Pilates® instructor in reformer, Cadillac, chair, barrels, and matwork, integrating Pilates-based movement into her practice to support strength, mobility, and mindful movement. Her background in dance has shaped her appreciation for body awareness and the art of efficient, controlled movement.
Her treatment style emphasizes hands-on therapy, patient education, and personalized programs tailored to each patient’s goals. Lauren is passionate about lifelong learning and building supportive, collaborative relationships that empower patients throughout their rehabilitation journey.
Lillian is a Physical Therapist Assistant and STOTT Pilates® Education Coordinator with a strong focus on bridging rehabilitation and wellness through movement-based care. She is a graduate of Amarillo College’s Physical Therapist Assistant program and holds a Bachelor of Applied Arts and Sciences from West Texas A&M University.
Lillian works closely with physical therapists and has extensive experience in outpatient orthopedics and pelvic floor rehabilitation. She has completed comprehensive STOTT Pilates® training in matwork, reformer, Cadillac, and chair, allowing her to integrate Pilates-based rehabilitation and performance training into patient care.

Tifany is the Front Office Manager at Re-Form with over 25 years of experience in healthcare administration and patient support. Her background as a medical assistant and certified phlebotomist allows her to bring both knowledge and compassion to every interaction.

In her role, Nikkia oversees new patient scheduling as well as therapy billing and insurance coordination, helping ensure a smooth and supportive experience for both patients and providers. She values the collaborative, positive environment at Re-Form and plays an integral role in daily operations.
