In the state of Texas, physical therapists may legally evaluate and treat patients within their scope of practice for up to 30 business days without a physician referral.
Re-Form offers direct access and welcomes patients without a referral. We also value an interdisciplinary approach to care and are happy to communicate with your physician or healthcare provider as needed—regardless of the duration of your therapy plan.
Re-Form offers both insurance-based and self-pay physical therapy options. We are currently in-network with most major insurance providers, including Medicare.
