Physical Therapy Pricing

Re-Form Physical Therapy focuses on results-driven recovery and long-term movement health. Whether you’re healing from injury, dealing with chronic pain, or working toward performance goals, our approach is hands-on, personalized, and designed for real progress.

In the state of Texas, physical therapists may legally evaluate and treat patients within their scope of practice for up to 30 business days without a physician referral.

Re-Form offers direct access and welcomes patients without a referral. We also value an interdisciplinary approach to care and are happy to communicate with your physician or healthcare provider as needed—regardless of the duration of your therapy plan.



Do I Need a Physician Referral?

Re-Form offers both insurance-based and self-pay physical therapy options. We are currently in-network with most major insurance providers, including Medicare.

Self-pay pricing is listed above for clients who prefer to pay out-of-pocket.
